Do I need to give a phone number?
No. The quiz does not ask for a phone number. After the result, you can choose to open WhatsApp and send the prepared callback request.

The Vital-Check form asks for your first name (optional), email address and answers about everyday routines, including food, sleep, movement, stress, daylight and supplements. The weight-goal question is optional. Please do not share diagnoses or medication details.
Form answers are processed for the Vital-Check you expressly requested. The detailed evaluation is sent by email after the required consent.
For the PDF guide, an email address is required and a first name is optional. After successful submission, the download appears on the page; this guide flow does not include a follow-up email.
Required consent covers only the Vital-Check you requested. The optional follow-up email choice is separate. Email 2 is only a short, friendly check-in question, with no recruiting or partnership pitch. The same person receives partnership information only after replying to Email 2 with the standalone word “Team”. That reply is not consent to unrelated marketing.
